Northern Health are seeking applications from motivated and suitably experienced Clinical Nurse Consultant to join the Department of Respiratory and Sleep Medicine.
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.
Services are provided through our six main campuses:
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.
About the Role:
This role is focused on improving outcomes for patients living with Chronic Obstructive Pulmonary Disease (COPD) and other chronic respiratory conditions through innovative, patient-centred models of care.
Working as part of a dedicated team of Respiratory CNCs, you will provide advanced clinical leadership, care coordination, education and support across both inpatient and outpatient settings.
A key component of the position involves coordinating and delivering care within a Virtual Assisted Self-Management (ASM) Program, supporting patients through telehealth, remote monitoring and digital health technologies.
You will work collaboratively with multidisciplinary teams, community providers and patients to develop and implement evidence-based care plans that improve self-management, reduce avoidable hospital presentations and enhance quality of life.
This is an opportunity to contribute to the development of innovative digital care pathways, lead service improvement initiatives, support clinical education and play a pivotal role in shaping the future of respiratory care delivery.
