Northern Health are seeking applications from motivated and suitably experienced Diabetes Nurse Educator to join the Diabetic Nurse Education team. 


 


Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.


 


Services are provided through our six main campuses:


Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals and Kilmore District Hospital.  In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.


 


About the Role:


The Diabetes Educator provides comprehensive, patient-centred care to individuals with diabetes and their carers. The Diabetes Educator contributes clinical nursing expertise, skills in coordination of care and clinical and professional leadership within the multidisciplinary team. The role is pivotal in leading and supporting the provision of evidence-based management, research and service development to improve health outcomes in people living with all types of diabetes.


 


To be successful in this role you will have:



  • Specialist clinical experience at an advanced level in diabetes education and a demonstrated ability to deliver responsive evidence-based care

  • Recent inpatient management experience

  • Experience in chronic disease education and self-management

  • Clinical leadership experience

  • Experience in working in a large multi-campus organisation

  • Prior experience in research

  • Experience working with clients from culturally and linguistically diverse backgrounds

  • Insulin pump experience
