Northern Health are seeking applications from motivated and suitably experienced Midwife to join the Antenatal Clinic Team in the role of GP Liaison Midwife.
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.
Services are provided through our six main campuses:
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg.
About the Role:
The GP Liaison Midwife acts as a crucial link between a hospital's maternity services and general practitioners (GPs), facilitating communication, collaboration, and improved patient care. This role involves receiving referrals from the antenatal midwives for patients whom are suitable for GP Shared Care and then coordinating that care with the appropriate qualified Shared Care GPs. The role focuses on building relationships with GP's, enhancing shared-care policies and pathways, promoting hospital services, coordinating education and support, and ensuring timely and smooth patient pathways between the hospital and primary care.
