Northern Health are seeking applications from motivated and suitably experienced Chronic Disease Registered Nurse to join the Hospital Admission Risk Program (HARP) team. 


 


 


 


Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north. 


  


Services are provided through our six main campuses: 


Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals Centres and Kilmore District Hospital.  In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park, Mernda and Coburg. 


   


 


 


About the Role:


 


HARP are seeking an experienced Registered Nurse to join the Hospital Admission Risk Program (HARP) Chronic Disease team at Northern Health. 


 


 


 


This role is responsible for delivery of safe, high-quality care to clients with Chronic Disease (Heart Failure and/or respiratory disease) and/or complex needs who are at risk of readmission to hospital. 


  


 


To be successful in this role you will have: 


 



  •  Minimum 5 years clinical experience

  • Specialist clinical experience at an advanced level in Heart Failure and/or Respiratory Nursing 



  • Completed or Working towards Post Graduate advanced clinical nursing qualification.  



  • Demonstrated advanced clinical assessment, intervention planning and delivery skills. 



  • Demonstrated ability to deliver responsive, evidence-based care.  



  • Clinical Leadership in the continuing care setting 



  • Demonstrated high-level communication skills both oral and written  



  • Demonstrated high level problem solving and planning ability. 



  • Experience in Community Nursing
