Clinical Nurse - OPC

Provides totalindividualized patient care and assumes accountability forassessing, planning, implementing and evaluating care, dischargeplanning for each patient and assessing for current and potentialproblems. Accurately, concisely and appropriately documents care,interventions and outcomes. Contributes to the achievement ofclinical quality outcome indicators and patient satisfaction bydemonstration of extraordinary service. Utilizes a professionalpractice framework and evidence-based care, identifies improvementareas while minimizing risks creating care that is optimal for ourpatients and families. Promotes a safe environment and safe patientcare by embracing a safety culture and identifying areas of risk.Educates the patient/family and other members of the health careteam. Contributes to excellence in outcomes, a positive patientexperience and a healthy professional environment. Contributeseffective use of resources. Leads and/or contributes toservice-based Performance Improvement activities and organizationalcommittees as assigned. Develops and sustains (maintains) ownknowledge, clinical skills (mandatory training), professional(portfolio or profile) awareness and provides documented evidenceof performance and maintenance of skills consistent with position.Treats colleagues, patients, and visitors with dignity and respectat all times and promotes and maintains effective communication andteamwork. Maintains confidentiality at all times. Identifies andalerts members of the medical team to changes in medical status ofthe patient. Works in collaboration with Case Management to achievegoals. Participates in new staff/student orientation, assists inmaintaining a suitable learning environment and acts as apreceptor/mentor in the supervision and teaching of newstaff/students to develop their competence. Maintains aprofessional demeanor and upholds the organizational values at alltimes. Acts as a role model for junior staff and assists inpromoting and sharing knowledge via mentorship/preceptorshipprograms in line with relevant research and evidence basedpractice. Follows all Hospital related policies and procedures.Performs other related duties as assigned.
