JOB SUMMARY: The Clinical Pharmacist - I willbe responsible for providing pharmaceutical care services topatients within Sidras healthcare facility. The post holdercollaborates with healthcare professionals to optimize medicationtherapy and ensures safe and effective medication use. KEY ROLEACCOUNTABILITIES: • Conducts comprehensive medicationreviews to assess appropriateness, effectiveness, and safety ofmedication therapy. • Develops and implementspharmaceutical care plans for individual patients based on theirspecific needs and medical conditions. • Provides druginformation to healthcare professionals, patients, and caregivers.• Monitors patient outcomes and adjusts medication therapyas necessary to achieve desired therapeutic goals. •Participates in multidisciplinary healthcare teams to develop andimplement treatment protocols and guidelines. • Ensurescompliance with regulatory requirements and standards of practicein pharmacy operations. • Provides education and trainingto pharmacy staff, healthcare professionals, and patients onmedication-related topics. • Maintains accurate andcomplete patient records and documentation. • Staysabreast of developments in pharmacy practice, pharmacotherapy, andhealthcare trends. • Adheres to Sidra’sstandards as they appear in the Code of Conduct and Conflict ofInterest policies • Adheres to and promotesSidra’s Values QUALIFICATIONS, EXPERIENCE AND SKILLS:ESSENTIAL PREFERREDEducation • Bachelor of Pharmacy plus• PharmD or master's degree in clinical pharmacyor its equivalent All degrees must have been obtained from an ACPEaccredited college of Pharmacy or equivalent Experience •2+ years of post-graduate experience as a licensed registeredClinical Pharmacist in a, accredited hospital setting. (Pharmacyinternship period is not counted in the experience) • 0experience as fresh PharmD graduate from Qatar University Clinicalpharmacy experience in a pediatric hospital settingCertificationand Licensure • Licensed in Pharmacy in country of origin• Obtain State of Qatar Pharmacy License after onboarding.
