Responsibilities
- Embracesthe philosophy of our company to render care in a manner, approachand with the skill that promotes maintenance or enhancement of eachpatients clinical status and quality oflife.
- ConductAnnual Wellness Visits on all long-term Nursing Facility residentsand may perform Annual History and Physicals as requested byFacilities.
- Conductclinical interventional and episodic direct care visits, andappropriate re-evaluation of carevisits.
- Provideongoing evaluation and treatment of chronic, co-morbid illnesses,as medicallyappropriate
- Evaluate/treatresidents for changes incondition.
- Reviewmedications, orders, and diagnostic tests, and respond to clinicalabnormalities appropriately for all patients under their care, asallowed and/or requested by collaboratingphysicians.
- Coordinatecare with collaborating physicians as indicated for overall medicalmanagement.
- Assistwith facility-wide illnesses, where appropriate collaborationsexist.
- Coordinatecare with other disciplines, such as Therapists, Specialists, andSocialWorkers.
- Maintaincommunication with families/attend family conferences whenpossible.

Requirements
- ValidNew Mexico Nurse Practitioner or Physician Assistant license& current board certification in yourfield.
- Bachelorsand Masters Degree in Nursing or in an accredited PhysicianAssistantprogram.
- Experiencein nursing is required, and some experience as a NursePractitioner/Provider is preferred, but notnecessary,
- Demonstratednursing/NP critical thinking skills applicable to the aged,chronically ill, and convalescentresident.
- Strongleadership, interpersonal, and problem-solving skillspreferred.
- Excellentwritten and oral communicationskills
- Possesstraits of punctuality, organizational skills, and ability tofunction within ateam.
- Proofof complete COVID vaccination by first day of employment orqualifiedexemptions.
Valid New Mexico Nurse Practitioner or Physician Assistant license& current board certification in your field. Bachelors andMasters Degree in Nursing or in an accredited Physician Assistantprogram.
