Job Description:

• Involve in care management and cross referrals to specialists through dedicated protocols
• Ensure completion of screening, assessment, anticipatory guidance and psychoeducation for patients/ participants 
• Maintain a record of participants in relevant program and data collection accurately.
• Work with Research Assistant in data collection and surveys.
• Anu ad-hoc duties as per assigned. 

Requirement: 

• Minimum Diploma in Health Promotion or Sciences or equivalent.
