Overview:

Our Healthcare Institution client is looking for energetic and highly motivated Individual to join their fast-growing industry as Care Coordinator. The successful candidate will play a critical role in helping patients navigate the healthcare system and ensuring they receive the appropriate care and support they need.

📍 Working Location: West 
📆 Working days: 5.5 days (Mon to Fri Office Hours & Sat morning)
⏰ Working Hours: Office Hour
💰 Salary: Basic Up $2500 + AWS + VB + Other Attractive Benefits

Job Description:

🌟 Conduct necessary preventive health screenings and assessments, based on individual patient needs.

🌟  Identifying patients who would benefit from lifestyle modification education programs and refer or enrol them in care management programs that are tailored to their specific health needs.

🌟  Comfortable with using systems as need to document and track patient progress / schedule their appointments for the programmes.

🌟  Assist in in planning, organizing, and coordinating health promotion activities for staff members at the workplace, and will also be expected to carry out any additional duties assigned by your supervisor.

🌟  Conduct and analyse survey gathered from users’ feedback

Job Requirement:

🎓 Minimum GCE 'O' Level

🎓 Diploma with qualification in Health Promotion or Sciences will have an advantage

📊 Experience in healthcare will be advantageous

🖥️ Proficient in Microsoft Office applications 

🏥 Self-motivated and has interest in patient care.

🗣️ Good communication and coordination skills

💡 Proactive and a team player
