You will be responsible for the Clinical Care and Coordination for the patients and assists to establish the Department and Divisional clinical database for outcome tracking.
Job Responsibilities
You will be responsible for the following:
- To coordinate and conduct clinical quality patient assessment for inpatient and outpatient where required. Assist clinicians to ensure prompt replies and reply rate to assessments required.
- To set up and maintain clinical database for clinical outcome tracking.
- To closely monitoring and review of patients’ treatment progress using the clinical pathway where applicable.
- To collaborate with patient, patient’s next-of-kin, wards, clinics and operating theatre to improve clinical outcome and patients’ satisfaction.
- To analyze and work on areas for improvement with consultants.
- Keep track of OT slots / bookings / utilization and maintain good communication with clinic manager and financial counsellor. Also, keep track of OT cancellations and reasons
- Provides patient/family information and education on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, exercise & coping strategies at home following discharge.
- Cross disciplinary collaboration i.e. with oncology/radiology department to ensure that the necessary diagnostic tests are scheduled and actualized.
- To present data on various wait time i.e. Wait time for admission, disclosure of diagnosis, first visit for surgeons / medical oncologist / radiation oncologist / palliative oncologist, start of the various treatments every quarter.
- Coordinate and ensure the attendance of relevant stakeholders at the meeting for auditing and process improvement purposes.
Requirements
- Degree in Biomedical Sciences, or Physiotherapy, or State-Registered Nurse with diploma/post- basic qualification in medical/surgical nursing.
- Ability to multi-task and work under tight timelines.
- Good analytical and organization skills.
- Excellent communication, problem solving, presentation, negotiation, and interpersonal skill.
