My client is a Renowned Healthcare Organization looking for Social Service Assistant

• 5 Days / Office Hour / Novena / Rochor 
• Salary up to $2800
• Diploma in any field

Responsibilities

• Interviews and provides information to patients and advising prevention                                 
• Contact traces of patients diagonsed.                       
• Follow-up on patients who have been diagnosed 
• Contact patients for screening and treatment.                    
• Reviews electronic medical records (EMR) details, follow-up with doctor i/c and codes diagnosis.                 
• Reviews treatment details and updates outcomes                              
• Provides Medical Scheme information and education to Medical Scheme registrants.                                 
• Participates in public health, educational and research activities and projects 

Requirements

• Minimum Diploma in any discipline (preferably in social work)                                                                                         
• Counselling background preferred.
• Experience in social service is an advantage
• Comfortable to work in the healthcare setting
• Good interpersonal and communication skills
