Job Description - Senior / Executive, Outreach & Community Health

S3 aims to provide the best quality of post-stroke care in the community so that our clients and their families are able to rebuild their lives after stroke. Beyond rehab, the Wellness Team plays a critical role in this continuum of care by creating a safe and supportive environment for stroke survivors and their families to enhance their physical, mental, social, and emotional health.

Responsibilities

Outreach and Community Health

  • Main job role of supporting and fulfilling the requirements of S3’s Active Ageing Centre services, which includes but is not limited to:
    • Conducting outreach activities to seniors living within S3’s Boon Lay service area
    • Monitoring and tracking senior’s wellbeing and participation
    • Conducting regular visits to frail and homebound seniors
    • Planning and coordinating activities for the seniors with internal and external stakeholders.
    • Generating and maintaining reports
  • Liaise with Community Stakeholders such PA, NUHS, GP clinics, etc to organize and conduct outreach activities including roadshows, talks, wellness programmes, events, and workshops.
  • Recruit and work closely to train and coordinate volunteers for the outreach and programme activities
  • Support the volunteer management team in all aspects as needed

Wellness and Community Health Team

  • Staff will be a member of the Wellness and Community Health Team, which oversees both the S3 Wellness Programme and AAC
  • Staff will support activities within the team as required and also organisational events.
  • To support any other organisational needs and requirements as assigned.

Activity Coordination

  • Work with internal and external stakeholders to co-ordinate activities for the seniors and recruit seniors to participate.

Requirements

  • Minimally diploma in any discipline.
  • Diploma in Social Science or Healthcare related course would be advantageous.
  • Positive attitude towards tasks and objectives.
  • Meticulous and organized.
  • Ability to communicate effectively, possessing good stakeholder management skills.
  • Excellent project management and events coordination skills.
  • Familiar with Microsoft Office applications experience will be advantageous.
