Nursing Clinical Facilitator

The Main Purpose of the Job:

To complete ad-hoc risk assessments for various homecare clients.

Responsibilities & Competencies:

• - Client visits with Homecare clients


• - Reporting & Documenting 


• - Updating care plans


• - Must be strong administratively


• - Good Eye for detail


• - Excellent patient care experience


• - Ability to prevent or limit risk


• - Ability to understand client needs


• - Good communication and interpersonal skills capable of maintaining strong relationships.


• - Strong organizational and multi-tasking skills.


• - Excellent analytical and problem-solving abilities.


• - Must be willing to travel with a valid Drivers License  

Required Education:

Degree / Diploma in Nursing

Desired Education:

Nursing Education Diploma; Unit specific certificate or diploma where appropriate (e.g. Emergency Centre / Operating Theatre / Critical Care Unit)

Essential Minimal Requirements:

1 year of Clinical Experience as a professional nurse

Desired Experience

1-2 years experience as a peer mentor; 1-2 years experience as a workplace assessor.
