QUALIFICATIONS
Graduate from an accredited physical therapy Preferably two years of clinical experience (if applicable). Capable of lifting the equivalent weight of an adult-size Licensed by the state in which the Physical Therapist resides or practices Physical Therapy according to state

PHYSICAL DEMANDS
Requires moderately heavy physical exertion on a regular and reoccurring basis such as extensive driving, assisting patients in transfer activities (wheelchair to bed, to tub, to commodes) and providing substantial support to individuals in ambulation Regularly required to use hands and Frequently required to stand, walk, reach with hands and arms, stoop, kneel, crouch, or Occasionally required to Occasionally lift and/or move over 100 Specific vision abilities include close, peripheral, depth perception and ability to adjust

ESSENTIAL FUNCTIONS
Provides treatment as ordered by the attending Interprets, plans, establishes, and exercises proper treatment program in accordance with the physician s referral, periodically, reviewing the patient s program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy Assists the physician in evaluating disability, injury, or disease process of individual patients and in evaluating the patient s level of


Contacts the physician directly for patient reports and care as Observes, records, and reports to the physician the patient s reaction to the therapy program or any changes in the patient s Submits to the physician and the Program Supervisor appropriate written reports to the patient s progress at the designated intervals using appropriate and designated

Participates in staff development activities and in in-services education


Demonstrates evidence of continuing education conducted for in-service
