PHYSICALDEMANDS
Requiresmoderately heavy physical exertion on a regular and reoccurringbasis such as extensive driving, assisting patients in transferactivities (wheelchair to bed, to tub, to commodes) and providingsubstantial support to individuals in ambulation Regularly requiredto use hands and Frequently required to stoop, kneel, crouch, orOccasionally lift and/or move over 100 Specific vision abilitiesinclude close, distance, depth perception and ability to adjust

ESSENTIALFUNCTIONS
Provides direct physical therapyservices according to directions of the Physical Therapist and inaccordance with the physician s plan of Observes, records andreports to the Physical Therapist and clinical supervisor responsesto treatment and any changes in the patient s

Treats the patient in accordance with the care planafter the PT performs the initial evaluation to establish treatmentgoals, frequency, duration and specific modalities to be used inaccordance with the physician s orders with the Treat patients torelieve pain, develop or restore function and maintain maximumperformance, using physical means, such as exercise, massage, heat,light, and

Reviews weekly the patient sContacts Physical Therapist directly for patient reports and careas needed, or required by State/Federal

Submits appropriate written reports of the patient s progress atthe designated intervals to the physician and/or All notes writtenby the PTA will be co-signed (if state requires) by the supervisingPhysical

Instructs patients regardingphysical therapy procedures designated by the supervising Physical

Adheres to all applicable federal, state,local and company-maintained standards of care and ethics andpolicies concerned with the practice of physical therapy and amongother things, personnel

Participates instaff development activities and in in-services education

Meets productivity and utilizationexpectations as established by the Company and the
