Key Responsibilities

• Conduct detailed assessments, diagnosis, and management of patients with chronic diseases.
• Develop and implement individualized care plans aimed at disease control and prevention of complications.
• Provide patient education to promote self-management and adherence to treatment regimens.
• Collaborate with healthcare professionals to coordinate comprehensive care and optimize outcomes.
• Monitor patient progress and make adjustments to treatment plans as necessary.
• Maintain accurate and thorough medical documentation in compliance with regulatory standards.
• Participate in continuing medical education and stay updated with advances in chronic disease management.

• Medical degree specializing in Internal Medicine or equivalent.
• Certification or specialization in chronic disease management is preferred.
• Valid medical license to practice in Abu Dhabi/UAE.
• Proven clinical experience in managing chronic diseases within internal medicine settings.
• Strong diagnostic, clinical, and patient education skills.
• Excellent communication and teamwork abilities.
