Adult Qualified Social Worker for Assessments

• Manage and prioritise workload effectively to meet deadlines while maintaining a high standard of written documentation and ensuring the delivery of impactful interventions and positive outcomes for service users.

• Conduct assessments and reviews in a person-centred, holistic manner, focusing on the individual's strengths and exploring preventative measures that enhance their independence and well-being.

• Provide ongoing support to carers and family members, ensuring they are actively involved in the assessment and care planning process, while addressing their needs and concerns.

• Ensure all assessments and reviews are in compliance with the Mental Capacity Act, making informed judgments about individuals who may experience significant challenges engaging in the process. In such cases, ensure the appropriate advocacy support is in place.
