Contract role, Care Act Assessment Team 

Pertemps are recruiting for an experienced Adult’s Social Worker in Herefordshire 
This is to work in a Care Act Assessment Team 
This is to carry out short term assessment & support planning with service users throughout Herefordshire who have recently left the hospital via Discharge to Assess and who may require ongoing services.  

There is plenty of hybrid working available.
