Adult Social Worker - Discharge to Assessment D2A

Pertemps social care are supporting Redbridge London Borough Council by recruiting for an experienced social worker. Working in a fast paced team environment you will be committed to improving the care of vulnerable adults. You will need to be able to work well in a team setting and have excellent interpersonal and written skills.

The post of a social worker is to support the discharges of residents of the borough from the local acute, out of borough hospitals and the local Rehabilitation Unit.
The team works in line with the Discharge to Assess model of care to arrange services to assist with the recommended discharge pathways. The applicant should have previous experience in adult social services, with a keen interest in hospital discharges. Do you have strong skills in multidisciplinary work?
Location: Ilford
Rate: £29 per hour
Hours: 36 per weekKey Responsibilities:

• Assessments
• Undertaking Mental Capacity Assessment
• Including excellent knowledge of the relevant social care legislation
• Safeguarding and of the continuing healthcare process

Candidate Requirements:

• A degree in Social Work
• Registration with Social Work England (SWE)
• 3 years post qualifying experience
