This is to work in a Integrated Discharge Team at Rotherham Hospital.

• Experienced Social Worker needed to work in an Integrated Discharge Team at Rotherham Hospital. The position is office based Monday-Friday.

• This is a fast paced environment supporting timely and safe discharges from hospital on various pathways. Experience of MCA and working in best interest is essential. Work will involve some Safeguarding Adults, supporting A&E and some support to mental health wards.
