Role Overview
Pertemps social care are supporting Redbridge Council by recruiting for an experienced social worker. Working in a fast paced team environment you will be committed to improving the care of vulnerable adults. You will need to be able to work well in a team setting and have excellent interpersonal and written skills.

The post of a social worker is to support the discharges of residents of the borough from the local acute, out of borough hospitals and the local Rehabilitation Unit.

The team works in line with the Discharge to Assess model of care to arrange services to assist with the recommended discharge pathways.

Key Responsibilities:
The applicant should have previous experience in adult social services, with a keen interest in hospital discharges.

Have strong skills in multidisciplinary work:

• Assessments
• Undertaking Mental Capacity Assessment
• Including excellent knowledge of the relevant social care legislation
• Safeguarding and of the continuing healthcare process

Candidate Requirements:

• Degree in Social Work
• A minimum of 3 years post qualifying experience
• Social Work England (SWE) registration
• Right to Work in the UK

You will need to be able to work well in a team setting and have excellent interpersonal and written skills.
