?? About the Role

You'll be joining a proactive and supportive Access and Wellbeing Team focused on prevention, enablement, and strength-based support. This includes medium to longer-term social work interventions, safeguarding enquiries, reassessments, and reviews.

?? Key Requirements

?? Must be available to be physically present in the office 3 days per week
?? Role includes:

• Home visits ?
• Joint working with other professionals ??
• Duty responsibilities ??

? About You:

• Qualified and progressed Social Worker, registered with Social Work England.
• Completing Care Act assessments ??
• Court of Protection and Human Rights assessments ??
• Undertaking Carers Assessments
