Social worker required for Hospital Discharge Neighbourhood Team based in the West/North West area of Leeds.
Managing discharges from out of area hospital, recovery hubs, temporary placements and reviewing care in people's own homes.

role is required for 12 weeks.

Knowledge of Leeds CIS Apex recording system is essential
