This is to work in a Hospital Team at the Bristol Royal Infirmary and Southmead Hospital.
The Role:

• As a social worker in Adult Social Care, you'll play a crucial role in assessing and addressing the diverse needs of our community, navigating the varied challenges our vibrant city provides. Your responsibilities will also include:

• Conducting assessments and developing tailored care plans for service users and carers, ensuring their unique needs and priorities are addressed.

• Advocating choice and control, you'll empower individuals to make decisions and manage risks, ensuring services are organised in alignment with their preferences and needs.

• Upholding professional standards continuously as our service evolves, you'll monitor changing needs, develop support plans, utilise resources, and ensure Social Care remains future-ready through creative use of assistive tech.
