This is to work in a Hospital team

Background:

The impact of this proposed project will be around the early identification of discharge pathways, collateral history from multiple sources contributing to early decision making and actions supporting discharge planning that can be done prior to NC2R decision.

Additionally, expectation setting for the patient, staff and partner organisations regarding discharge pathway and anticipated resource.

It is understood that whilst the number of patients with no criteria to reside may not reduce, the days that they are delayed will further reduce.

Outcomes described are measurable and any unintended consequences positive or negative will be reviewed as the project develops.

It will demonstrate a model that is measurable and will be used to further influence discharge services across SFT NHS FT upon review.

The proposal is that a small team of experienced staff from Wiltshire Council and SFT will undertake early assessments of patients over 65 across 3 wards who have been admitted within 48 hours to establish discharge proposals, identify complexity and set background work in motion either within the Trust or in the community, prior to the patient becoming NC2R and within the context of their EDD.

This will potentially decrease the delay post NC2R for these patients and provide data by which further development may be decided
