Adults Social Worker
Team: Mental Health
Location: Kirklees
Rate: £35
The role is for an experienced and qualified Social Worker.
Tasks of the role will involve working peripatetically across North and South Kirklees to complete Care Act assessments and reviews of adults 18+ within care home settings, completing Mental Capacity Assessments and Best Interest Decisions, attending and actively participating in Continuing Healthcare Integrated Care Board assessments and reviews, completing Safeguarding Responses and Enquiries and considering strength based and person centred options for an individual's care throughout.
The role will be 37 hours during the day covering Monday - Friday. Flexible working arrangements can be discussed with the line manager.
Regular workload management and supervision will be provided.
