Are you a driven and compassionate Social Worker passionate about supporting adults with learning disabilities through significant life transitions? We're seeking a dedicated professional to join our Transitions Team in Doncaster.

About the Role:

• Work directly with adults with learning disabilities, supporting them to navigate complex transitions such as moving to more independent living, changing support providers, or accessing new services.
• Develop and implement person-centered transition plans, ensuring seamless and positive outcomes.
• Collaborate closely with families, care providers, and other professionals to coordinate support and advocate for individual needs.
• Conduct thorough assessments and provide ongoing support and guidance to individuals during their transition journey.
• Contribute to a supportive and dynamic team committed to improving the lives of adults with learning disabilities.

Requirements:

• Registered with Social Care England.
• Minimum 3 years' post-qualification experience as a Social Worker.
• Experience working with adults with learning disabilities, ideally within a transition-focused setting.
• Excellent communication, problem-solving, and case management skills.
• A proactive and solution-oriented approach to supporting individuals through change.
• Commitment to person-centered care and ethical practice.
