Job Description - Aftercare Worker Leaving Care

Job Title: Aftercare Personal Advisor (2 Positions Available)
Contract: 3-month initial contract (minimum) with the option to extend
Hours: Full-time (Monday - Friday, 8:30 AM - 4:30 PM)
Location: Hybrid working (3 days in the office, flexibility to meet role requirements)

About the Role We are looking for two dedicated Aftercare Personal Advisors to join our team within Early Help & Children’s Social Care. Our mission is to provide the right support at the right time to children and families, ensuring young people leaving care receive the highest quality service possible. We are committed to supporting care leavers in their transition to independent living, helping them build practical skills, maintain relationships, and achieve their potential.

Key Responsibilities
  • Provide a professional, multi-agency social work service to young people leaving care.

  • Offer direct advice and support to care leavers in areas such as accommodation, income, health, education, and training.

  • Maintain regular contact with care leavers and assist in their development of practical living skills.

  • Advocate on behalf of young people with agencies and support them in building negotiation skills.

  • Assist young people in maintaining family relationships and developing social connections.

  • Regularly review and update Pathway Plans for young people.

  • Keep accurate records and reports in line with policies and procedures.

  • Support team development by sharing knowledge and participating in team activities.

  • Work flexibly, including potential evening and weekend work when required.

About You Essential Qualifications & Experience:

  • Experience working with young people in a support role.

  • Knowledge of The Children (Leaving Care) Act 2000 and The Children Act 1989.

  • Understanding of safeguarding, equality, and diversity issues.

  • Strong communication skills (verbal and written) and ability to maintain accurate case files.

  • Ability to work both independently and as part of a team.

  • Full UK driving licence and access to a vehicle for work purposes (please do not apply if you do not drive).

  • Enhanced DBS required.

Desirable:

  • NVQ Level 3 in Childcare/Social Care or equivalent qualification.

  • Experience working in a multi-agency setting.

Special Conditions
  • Flexibility to work out-of-hours (evenings and weekends if required).

  • Willingness to travel outside the Trafford area when necessary.
