Join our established Community Mental Health Team and support adults with complex mental health needs by:

• Undertaking comprehensive assessments in the community and clinic settings

• Developing and implementing personalised, recovery-focused interventions

• Supporting service users to develop skills for independent living and social inclusion

• Working closely with psychiatrists, social workers, CPNs and other professionals to deliver holistic care

• Contributing to care planning, risk assessments and discharge planning

• Maintaining accurate and timely clinical documentation

To be successful in this role, you’ll need:

• A current and valid HCPC registration

• Previous experience in mental health (community setting desirable)

• Strong knowledge of mental health conditions, recovery models, and risk management

• Excellent communication and teamwork skills

• A commitment to delivering person-centred, evidence-based care
