Knowledge & Experience

• We are looking for an enthusiastic, proactive nurse to work within our Community Nursing Team.
• You will be working autonomously, across the borough, to provide holistic, child and family centered care for children and young people with SEND, PMLD and complex health needs, ranging from 2 - 19 years of age.
• This is a dynamic role encompassing: Assessment, planning, delivery and evaluation of clinical nursing care; writing appropriate care plans to ensure safe and effective care delivery.
• Delivery of specialist nursing care, including but not limited to; gastrostomy management, wound care, suction.
• Delivery of specialist training for families and school support staff.
• Multi-disciplinary liaison to ensure continuity of care and attendance at multi- professional meetings, including child protection case conferences.
• Provision of support to families, including signposting to specialist services and completing onward referrals for care.
• Providing health advice for education, health and care plans.
