What you will do:

• Conduct assessments to identify the needs of children with disabilities, developing and implementing care plans accordingly.
• Work closely with families, providing advice and support to enable them to care for their children at home.
• Collaborate with a range of professionals from other sectors to ensure a holistic approach to the child's care and support.
• Manage a diverse caseload, maintaining accurate and up-to-date records on each case.
• Attend case conferences and reviews, contributing to decision-making processes for each child's welfare.
