Clinical Pharmacist

Job Title: Primary Care Pharmacist 

Location: Oxford

Salary: £28 - £45 per hour

Job Type: Temporary, Agency Contract

Role Overview: We are seeking a highly motivated and experienced Primary Care Pharmacist to join a dynamic team in Oxford on a temporary basis through an agency contract. The successful candidate will play a crucial role in providing pharmaceutical care to patients, ensuring the safe and effective use of medications, and supporting the overall healthcare team.

Key Responsibilities:

• Conduct medication reviews and provide expert advice on the use of medicines.
• Collaborate with healthcare professionals to develop and implement patient care plans.
• Monitor patient progress and adjust treatments as necessary.
• Educate patients and carers on medication management and adherence.

Requirements:

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Minimum of 2 years' experience in a primary care setting.
• Strong clinical knowledge and excellent communication skills.
• Ability to work effectively within a multidisciplinary team.
