Are you a compassionate, proactive, and skilled nurse looking to make a real impact in people's lives? We are currently seeking a Community Nurse to join our team, providing high-quality nursing care to patients in their homes and community settings across Spalding, Stamford, Market Deeping and Bourne!

About the Role:
As a Community Nurse, you will play a vital role in delivering patient-centred care to individuals with a wide range of health needs. You'll work independently and as part of a multidisciplinary team to assess, plan, implement, and evaluate care plans tailored to each patient.

Key Responsibilities:

• Provide nursing care to patients in their homes or community clinics

• Manage chronic conditions and support early discharge from hospital

• Support patients with complex health needs, including wound care, palliative care, and long-term condition management

• Collaborate with GPs, social workers, physiotherapists, and other healthcare professionals

• Promote health education and self-management strategies

• Maintain accurate and timely patient records

What We're Looking For:

• Registered Nurse (Adult) with valid NMC registration

• Full UK driving licence and access to a car (or suitable alternative transport)

• Excellent communication and clinical decision-making skills

• Ability to work independently and manage your own caseload

• Previous community or district nursing experience is essential
