Job Description - Community Occupational Therapist


5 Month Contract With A Local Authority











Job Summary:



  • We are seeking a dedicated and professionally qualified Community Occupational Therapist to work within a multidisciplinary setting to improve the independence, safety, and overall quality of life for adults in the community. The role involves direct engagement with service users, carers, and professionals from health and social care agencies to assess needs and deliver person-centred occupational therapy interventions, equipment recommendations, and environmental adaptations.

  • You will produce individual care and support plans, coordinate and present assessment findings to relevant multi-agency panels, and monitor progress to ensure the delivery of high-quality outcomes.









Key Responsibilities:



  • Meet with service users, carers, families, and professionals in various community settings (homes, clinics, care settings) to conduct functional assessments and identify goals.


  • Promote independence, rehabilitation, and wellbeing through person-centred planning, equipment provision, and home/environmental adaptations.


  • Develop and implement individual care and support plans, including strategies for rehabilitation and risk reduction.


  • Coordinate assessments and contribute to multi-agency case conferences, including preparing and presenting reports to panels (e.g., housing, funding, safeguarding).


  • Specify and recommend assistive equipment and minor/major adaptations to promote safety and functional ability.


  • Monitor and review intervention plans and adjust as necessary.


  • Work in accordance with relevant legislation, including the Care Act 2014, Housing Grants, and Health and Safety frameworks.


  • Maintain accurate, clear, and timely records of assessments, decisions, and interventions in line with professional and council standards.


  • Adhere to the Royal College of Occupational Therapists' Code of Ethics and HCPC Standards of Proficiency.


  • Take responsibility for managing own workload, meeting performance targets and deadlines, and escalating complex cases or workload issues to the Team Manager.



Essential Requirements:



  • Degree/Diploma in Occupational Therapy.


  • HCPC registration as an Occupational Therapist.


  • Experience working in community or social care settings with adults.


  • Knowledge of relevant legislation, including Care Act, Mental Capacity Act, and housing/adaptation policies.


  • Skilled in functional assessment, rehabilitation planning, and risk assessment.


  • Strong knowledge of equipment provision and home adaptations (e.g., Disabled Facilities Grants).


  • Effective written and verbal communication skills.


  • Ability to work collaboratively with multi-agency teams and independently manage a caseload.


  • Understanding of safeguarding responsibilities and data protection legislation.

  • Working Hours: 35 per Week
