Join our Community Mental Health Team as a Band 6 CPN, supporting individuals with complex mental health needs. Your key responsibilities will include:

? Managing a caseload of service users in the community.
? Conducting assessments, care planning, and delivering recovery-focused interventions.
? Working collaboratively with multidisciplinary teams, families, and carers.
? Providing psychoeducation and relapse prevention strategies.
? Administering medication and monitoring mental health stability.
? Safeguarding and risk management to ensure patient safety.
? Supporting transitions between services and signposting to relevant support networks.

To be successful in this role, you will need:

? A current and valid NMC registration as a Mental Health Nurse (RMN).
? Experience working within a Community Mental Health setting.
? Strong risk assessment and crisis intervention skills.
? Excellent communication and interpersonal skills.
? The ability to work independently and as part of a multidisciplinary team.
? A valid full UK driver’s license (preferred but not essential).
