Join our team as a Band 6 CPN or Social Worker and play a key role in improving the lives of adults with complex mental health needs in a community setting. In this role, you will:

• perform comprehensive patient assessments

• create and implement personalised care plans

• deliver evidence-based treatments to optimise patient outcomes

• collaboration with multidisciplinary teams to provide holistic care

• maintain accurate patient records

To be successful in this role, you will need:

• a current and valid NMC or SWE registration and pin

• experience working with adults with complex mental health needs

• strong communication and interpersonal skills

• the ability to work independently and within a multidisciplinary team

• a commitment to delivering high-quality patient care

• a full UK driving license and access to a vehicle

This role offers a flexible working position requiring availability from Monday to Friday, with flexibility where possible. The contract is for 3 months, with a strong possibility of extension based on performance and service needs.
