Join our Older Adults Community Mental Health Team (CMHT) as a Community Psychiatric Nurse (CPN) and make a meaningful impact in the lives of older adults experiencing mental health challenges. Your key responsibilities will include:

? Conducting comprehensive mental health assessments and developing personalised care plans.
? Providing specialist interventions for older adults with conditions such as dementia, depression, and anxiety.
? Working closely with families, carers, and multidisciplinary teams to provide holistic care.
? Administering depot injections, overseeing medication management, and supporting physical health monitoring.
? Delivering crisis intervention, relapse prevention, and recovery-focused care.
? Conducting risk assessments and safeguarding for vulnerable adults.
? Signposting and referring individuals to appropriate community services and inpatient care when required.

To be successful in this role, you will need:

? A current and valid NMC registration as a Mental Health Nurse (RMN).
? Proven experience working with older adults in a community mental health setting.
? Strong assessment, crisis intervention, and risk management skills.
? Excellent communication and interpersonal skills.
? The ability to work independently and as part of a multidisciplinary team.
? A valid full UK driver's license (preferred but not essential).

This is a Monday to Friday role from 9 AM - 5 PM, offering flexibility and a healthy work-life balance. The contract is for 3 months, with a strong possibility of extension based on performance and service needs.
