Join our team as a CPN and play a key role in supporting adults with mental health needs in the community. In this role, you will:

• Conduct comprehensive mental health assessments
• Develop and implement personalised care plans
• Provide evidence-based interventions and support to service users
• Work collaboratively with multidisciplinary teams and external agencies
• Maintain accurate patient records and ensure high standards of care

To be successful in this role, you will need:

• A current and valid NMC registration and pin
• Previous experience working as a CPN in an adult community mental health setting
• Strong assessment, care planning, and risk management skills
• The ability to work independently and as part of a team
• A full UK driving licence (preferred but not essential)

This is a full-time, onsite role in South Somerset, with an ongoing contract offering stability and flexibility.
