Join our Community Mental Health Team (CMHT) as a Band 6 CPN, providing vital support to adults with complex mental health needs. Your key responsibilities will include:

? Managing a caseload of service users, providing assessments and care planning.
? Delivering recovery-focused interventions to support service users in the community.
? Conducting risk assessments and crisis management to ensure patient safety.
? Working collaboratively with multidisciplinary teams, families, and carers.
? Administering medication management and monitoring mental health stability.
? Supporting transitions between services and signposting to relevant resources.

To be successful in this role, you will need:

? Current and valid NMC registration as a Mental Health Nurse (RMN).
? Experience working in a Community Mental Health Team (CMHT).
? Strong assessment, risk management, and crisis intervention skills.
? Excellent communication and teamwork abilities.
