Job Title: Social Worker - Connected Carers Team

About the Connected Carers Team:

The Connected Carers Team assesses and supports family members, friends, or others with a close connection to the child who step in to provide care.

Key Responsibilities:

• Assess and support family members, friends, or others with a close connection to the child who step in to provide care.
• Complete Form K and Special Guardianship Order (SGO) assessments.
• Offer ongoing supervision, advice, and training to carers.
• Work within the Signs of Safety practice model and use Liquid Logic to support evidence-based decision-making.
• Be part of a team recently rated "Outstanding" by Ofsted, with strong leadership and a supportive, flexible working culture.
