Forensic Social Worker - London Based

Join our team as a Forensic Social Worker and play a key role in improving the lives of patients preparing for discharge from secure inpatient settings. In this role, you will:

• perform comprehensive risk assessments, including risk of reoffending

• create and implement risk management and discharge plans

• deliver evidence-based care coordination to support recovery

• collaborate with multidisciplinary teams, local authorities, and safeguarding boards

• maintain accurate patient records and produce formal reports for panels and tribunals

To be successful in this role, you will need:

• a current and valid SWE registration

• experience working with adults in secure or forensic inpatient settings

• strong communication, safeguarding, and care planning skills

• the ability to work independently and within a multidisciplinary team

• a commitment to delivering high-quality, person-centred care

This is a full-time, Monday to Friday role from 9am to 5pm. The contract is for 3 months, with a strong possibility of extension based on performance and service needs.
