Are you a highly skilled and motivated Social Worker dedicated to supporting timely and effective hospital discharges? We are seeking a compassionate and experienced professional to join our Hospital Discharge Team in Shropshire.

About the Role:

• Work collaboratively with healthcare professionals, patients, and their families to facilitate safe and efficient transitions from hospital to community settings.
• Conduct comprehensive assessments, identifying individual needs and developing person-centered discharge plans.
• Liaise with various agencies and services to coordinate care and ensure access to appropriate support in the community.
• Advocate for patients' rights and ensure seamless continuity of care.
• Contribute to a positive and supportive team environment focused on achieving excellent patient outcomes.

Requirements:

• Registered with Social Care England.
• Minimum 3 years' post-qualification experience as a Social Worker.
• Experience working within a hospital setting or with discharge planning is highly desirable.
• Excellent communication, assessment, and case management skills.
• Strong understanding of health and social care legislation and pathways.
• A proactive and solution-oriented approach to problem-solving.
