Join our team as a Community Diabetes Dietitian and make a real difference in the lives of patients managing diabetes in a community setting. Your core responsibilities include:

• Conducting comprehensive dietary assessments for patients with diabetes.
• Developing and implementing personalised nutrition plans.
• Providing education and support to patients to optimise diabetes management.
• Collaborating with multidisciplinary teams to enhance patient outcomes.
• Keeping accurate and up-to-date records for compliance and continuity of care.
• Staying informed on the latest research and best practices in diabetes dietetics.

Requirements & Qualifications:
• Current and valid HCPC registration.
• Experience in community dietetics and diabetes management.
• Clinical NHS experience at Band 7 level.
• Strong communication and interpersonal skills.
• Ability to work both independently and as part of a multidisciplinary team.
• Commitment to delivering high-quality patient care.

Working Hours & Contract Duration:
• Schedule: Full-time
• Contract Duration: 3 months, with a strong possibility of extension based on performance and service needs.
