Join our team as a Community Dietitian and make a real difference in the lives of patients requiring home enteral feeding. Your core responsibilities include:

• Conducting specialist nutritional assessments for patients requiring HEF.
• Developing and implementing individualised feeding plans.
• Providing education and support to patients, families, and carers.
• Collaborating with multidisciplinary teams to optimise patient care.
• Keeping accurate and up-to-date records for compliance and continuity of care.
• Staying informed on the latest advancements in HEF and community dietetics.

Requirements & Qualifications:
• Current and valid HCPC registration.
• Experience in Home Enteral Feeding (HEF) and community dietetics.
• Clinical NHS experience at Band 6 level.
• Strong communication and interpersonal skills.
• Ability to work both independently and as part of a multidisciplinary team.
• Commitment to delivering high-quality patient care.
