Are you a PCN Pharmacist looking for a rewarding locum role in Herefordshire? We have an exciting opportunity for you!

• Job Title: PCN Pharmacist

• Location: Herefordshire

• Duration: Ongoing

• Schedule: Monday to Friday, 9am - 5pm with a 30-minute break

• Start Date: June/July 2025

• End Date: Ongoing

• Rate of Pay: £30-£40 per hour DOE

What You'll Be Doing:
Join our client's Primary Care team as a PCN Pharmacist and make a real difference to patient care in the local community. In this role, you will:

• Carry out clinical medication reviews for patients with long-term conditions

• Prescribe independently as part of the multidisciplinary team

• Provide expert advice on medicines optimisation

• Collaborate with GPs, nurses and other clinicians to deliver high-quality care

• Keep accurate and detailed patient records

Requirements & Qualifications:
To be successful in this role, you will need:

• GPhC registration as a Pharmacist

• Independent Prescriber qualification (essential)

• Minimum of 2 years' experience in Primary Care settings

• Excellent communication and clinical decision-making skills

• A proactive and flexible approach to patient care

Working Hours & Contract Duration:
This is a full-time, on-site role working Monday to Friday. While this role starts as fully on-site, there is potential for hybrid/remote working in the future based on performance and service needs. The contract is ongoing with scope for extension.
