Hunter Gatherer AHP have a fantastic locum opportunity for an experienced Primary Care Network Pharmacist to join a well-established service based in Surrey. If you're passionate about improving patient outcomes in a primary care setting and have a strong background in chronic disease management – particularly diabetes – this could be the perfect role for you.

Position: Locum Primary Care Network Pharmacist
Location: Surrey
Contract: Ongoing locum assignment
Start Date: June 2025
Schedule: Monday – Friday, 9am – 5pm
Hourly Rate: £32.00 - £42.00 DOE

What’s Involved:

You’ll become a vital part of a multidisciplinary team, delivering impactful, patient-focused care. Key responsibilities include:

• Conducting in-depth clinical reviews and managing long-term conditions

• Developing tailored care plans for patients with complex needs

• Supporting diabetes management initiatives across the PCN

• Working collaboratively with GPs, nurses, and other healthcare professionals

• Ensuring accurate documentation and continuity of care
  
  

Who We’re Looking For:

• GPhC-registered Pharmacist

• Independent Prescriber (IP) qualification

• Strong experience in primary care or GP practice settings

• Proven ability to manage patients with diabetes and other LTCs

• Confident working autonomously and within a wider team

• Committed to clinical excellence and continuous improvement
