Job Description - Outreach Recovery Practitioner

Job Title: Outreach Recovery Practitioner
Location: South East London
Rate: £22 - £23 P/H Ltd Umbrella (£20.33 PAYE Inclusive)

Responsibilities:

Conduct outreach shifts in all weather conditions and at various times, including early mornings, nights, and weekends
Engage with individuals experiencing rough sleeping and those involved in street-based activities
Collaborate with partner agencies to help individuals access accommodation, health services, substance use support, and other essential services
Accurately document interactions with individuals and professionals
Work flexibly across different operational sites and be available during unsociable hours, weekends, and severe weather conditions
Continuously improve personal performance, knowledge, and skills through training and development opportunities
