Job Purpose:

We are seeking a dedicated and experienced Physiotherapist to join our client's team. The role involves contributing to holistic assessments for adults with complex physical and medical needs, including those at risk of falls, and with neurological, musculoskeletal, and frail conditions. The primary goal is to work with key stakeholders and community services to avoid hospital admissions and assist in the timely discharge of those receiving care, providing support for discharge planning and oversight to prevent avoidable re-admissions.

Key Responsibilities:

  • Manage clients over the age of 18 requiring support discharge from acute settings, particularly those with complex health and social needs.
  • Provide advanced physiotherapy assessments, treatment, and evaluation of clients’ needs to avoid unnecessary hospital admissions and ensure swift, appropriate discharges.
  • Work within a multidisciplinary team (MDT) to avoid inappropriate admissions to long-term care.
  • Educate and support other healthcare professionals, carers, and students in the holistic management of clients with physical disabilities and cognitive impairment.
  • Supervise and mentor rehabilitation and healthcare assistants and physiotherapy students on clinical placements.
  • Participate in audit and research, disseminating findings and recommending changes to practice.
  • Contribute to service development and policies, providing specialist advice to clients and other professionals as required.

Clinical Responsibilities:

  • Deliver comprehensive holistic therapy assessments, setting short and long-term goals to maximize patient independence leading to safe discharge.
  • Undertake comprehensive needs-led assessments of patients with complex problems, working collaboratively with GPs and other professionals.
  • Provide specialist advice and support across the organization regarding the diagnosis and management of clients with multiple and complex disabilities.
  • Facilitate education and skills sharing among rehabilitation assistants and healthcare assistants.
  • Liaise closely with other professionals in the best interest of clients.
  • Monitor accurate and comprehensive patient records in line with professional standards and policies.
  • Demonstrate effective communication skills to deliver patient-focused care and establish therapeutic relationships in complex cases.
  • Manage clinical risk within your own caseload and interpret clinical and non-clinical facts to form accurate diagnoses and prognoses.

Requirements:

  • Proven experience in managing complex physical and psychological symptoms across a variety of conditions.
  • Ability to work autonomously within a multidisciplinary team.
  • Strong leadership, support, and advisory skills within an MDT.
  • Commitment to service development and strategic planning using specialist knowledge.
  • Excellent communication, reasoning, and negotiation skills.

What We Also Need:

  • Experience working in a community setting.
  • Ability to work autonomously.
  • A valid driving license and access to a car.

If you are passionate about making a difference in the lives of patients with complex needs and have the skills and experience required, we would love to hear from you. Please apply using the link below and we will be in touch to discuss the next steps of the application!
