• Schedule: Monday to Friday, 9am-5pm

• Rate of Pay: £30-£40 per hour (DOE)

What You'll Be Doing:

Join our client's team as a Primary Care Pharmacist and play a key role in supporting medication optimisation and chronic disease management within a GP practice setting. In this role, you will:

• perform comprehensive patient assessments

• create and implement personalised care plans

• deliver evidence-based treatments to optimise patient outcomes

• collaborate with multidisciplinary teams to provide holistic care

• maintain accurate patient records
  
  

Requirements & Qualifications:

To be successful in this role, you will need:

• a current and valid GPhC registration

• experience working in primary care or GP practice settings

• strong communication and interpersonal skills

• the ability to work independently and within a multidisciplinary team

• a commitment to delivering high-quality patient care
  
  

Working Hours & Contract Duration:

This role offers a full-time position requiring availability from Monday to Friday. The contract is ongoing with a strong possibility of extension based on performance and service needs.
