Locum Social Worker - Adult Community Team (Older People & Physical Disabilities) - Salford

Responsibilities:

• Provide essential support to older people in the community with mental health issues, physical and learning disabilities.
• Conduct Care Act assessments, develop and review care plans.
• Involvement in DSTs and court work where required.
• Manage a complex caseload while ensuring individuals receive the support they need.

To be considered for the role, ideally you will have:

• Minimum 2 years' experience in a similar social work setting.
• Diploma/Degree in Social Work.
• Registered with Social Work England.
• Proven experience handling complex cases, Care Act assessments, and DSTs.
